Home Care Liaison
Description Summary:
- The home care liaison (HCL) is responsible for the overall census growth of the agency through the development, implementation and evaluation of the agency marketing plan.
- Generate appropriate patient referrals/admissions from customers and continue to grow the number of referrals/admissions over time.
- Serve as pivotal liaison between customers (patients and referral sources) and Life Care at Home staff members.
- Represent Life Care at Home and its services in a competent professional and responsive manner.
- This position reports to the agency director.
Education and Experience Summary:
- Bachelor’s degree in Marketing, Business or related field preferred; or LPN/RN with extensive case management and/or discharge planning experience; or PT/OT/ST
- Familiarity with Professional Community preferred
- One or more years of sales experience with proven results desirable. Prior experience in healthcare setting is a plus.
