Registered Nurse Case Manager

Description Summary:

• The registered nurse is responsible for the clinical care of assigned home care patients which may include, but not be limited to, case management, specific nursing tasks, patient assessment, patient/family education, and coordination with the multidisciplinary team including RN, LPN, PT, PTA, OT, COTA, SLP, MSW and home health aides.
• The RN is responsible to report changes in condition to the physician and to the clinical coordinator as appropriate.
• The position reports to the clinical coordinator.

Education and Experience Summary: 

• Must possess current RN license in good standing in the state in which agency is located.
• Home health experience preferred
• Minimum of 1 year experience as a RN
• OASIS knowledge preferred
• Must have CPR certification
• Valid driver’s license and auto liability insurance with reliable transportation to travel to patient homes per patient schedule and agency needs.
