Physical Therapist


Description Summary:

  • The physical therapist provides rehabilitative care for patients needing assistance and training in ambulation, transferring, gait training and home exercise programs.
  • The PT works as part of the interdisciplinary team including RN, LPN, PT, PTA, OT, COTA, SLP, MSW and home health aides.
  • The PT reports to the rehab director or clinical coordinator.

 

Education and Experience Summary:

  • Graduate from a physical therapy curriculum approved by either the APTA, Committee on Allied Health Education and Accreditation of the AMA, or Council on Medical Education of the AMA and APTA
  • Minimum of 1 year experience as a PT
  • Home health experience preferred
  • Able to evaluate patient needs through ongoing assessment and revise care plans based on changes in patient condition
  • Must have CPR certification
  • Valid driver’s license and auto liability insurance with reliable transportation to travel to patient homes per patient schedule and agency needs.
