Physical Therapist
Description Summary:
- The physical therapist provides rehabilitative care for patients needing assistance and training in ambulation, transferring, gait training and home exercise programs.
- The PT works as part of the interdisciplinary team including RN, LPN, PT, PTA, OT, COTA, SLP, MSW and home health aides.
- The PT reports to the rehab director or clinical coordinator.
Education and Experience Summary:
- Graduate from a physical therapy curriculum approved by either the APTA, Committee on Allied Health Education and Accreditation of the AMA, or Council on Medical Education of the AMA and APTA
- Minimum of 1 year experience as a PT
- Home health experience preferred
- Able to evaluate patient needs through ongoing assessment and revise care plans based on changes in patient condition
- Must have CPR certification
- Valid driver’s license and auto liability insurance with reliable transportation to travel to patient homes per patient schedule and agency needs.
