Medical Social Worker – Master’s Level


Description Summary:

  • The medical social worker is responsible for the socio-economic assessment of the patient to include financial status, caregiver status, safety, living arrangements, family dynamics, and need for community resources.
  • The MSW is responsible to report patient findings to the interdisciplinary team including RN, LPN, PT, PTA, OT, COTA, MSW and home health aides.
  • The MSW reports to the clinical coordinator

 

Education and Experience Summary:

  • Master's degree from a school of social work accredited by the Council on Social Work Education
  • Minimum 1 year experience in social work in healthcare setting
  • Home Health experience preferred or one year experience in health care setting preferred
  • Must have CPR certification
  • Valid driver’s license and auto liability insurance with reliable transportation to travel to patient homes per patient schedule and agency needs.

 

Medical Social Worker – Bachelor’s Level


Description Summary:

  • The medical social worker (BSW) is responsible for the socio-economic assessment of the patient to include financial status, caregiver status, safety, living arrangements, family dynamics, and need for community resources.
  • The BSW is responsible to report patient findings to the interdisciplinary team including RN, LPN, PT, PTA, OT, COTA, MSW and home health aides.
  • The BSW reports to the clinical coordinator

 


Education and Experience Summary:

  • Baccalaureate  degree from a school of social work, psychology, sociology or other field related to social work
  • Minimum 1 year social work experience in a health care setting.
  • Home health experience preferred
  • Must have CPR certification
  • Valid driver’s license and auto liability insurance with reliable transportation to travel to patient homes per patient schedule and agency needs.
