Medical Social Worker – Master’s Level
Description Summary:
- The medical social worker is responsible for the socio-economic assessment of the patient to include financial status, caregiver status, safety, living arrangements, family dynamics, and need for community resources.
- The MSW is responsible to report patient findings to the interdisciplinary team including RN, LPN, PT, PTA, OT, COTA, MSW and home health aides.
- The MSW reports to the clinical coordinator
Education and Experience Summary:
- Master's degree from a school of social work accredited by the Council on Social Work Education
- Minimum 1 year experience in social work in healthcare setting
- Home Health experience preferred or one year experience in health care setting preferred
- Must have CPR certification
- Valid driver’s license and auto liability insurance with reliable transportation to travel to patient homes per patient schedule and agency needs.
Medical Social Worker – Bachelor’s Level
Description Summary:
- The medical social worker (BSW) is responsible for the socio-economic assessment of the patient to include financial status, caregiver status, safety, living arrangements, family dynamics, and need for community resources.
- The BSW is responsible to report patient findings to the interdisciplinary team including RN, LPN, PT, PTA, OT, COTA, MSW and home health aides.
- The BSW reports to the clinical coordinator
Education and Experience Summary:
- Baccalaureate degree from a school of social work, psychology, sociology or other field related to social work
- Minimum 1 year social work experience in a health care setting.
- Home health experience preferred
- Must have CPR certification
- Valid driver’s license and auto liability insurance with reliable transportation to travel to patient homes per patient schedule and agency needs.
