Licensed Practical Nurse (LPN/LVN)

Description Summary:

• The licensed practical nurse is responsible for the clinical care of assigned home care patients which may include, but not be limited to, wound care, venipuncture, monitoring vital signs, medication education and administration.
• Responsible to report changes in patient condition to the RN case manager
• The position reports to the clinical coordinator.

Education and Experience Summary:

• LPN license in good standing with the state where agency is located
• Home Health experience preferred
• Minimum of 1 year clinical experience as a LPN
• Must have CPR certification
• Valid driver’s license and auto liability insurance with reliable transportation to travel to patient homes per patient schedule and agency needs.
