Intake Coordinator

Description Summary:

• The intake coordinator position encompasses intake coordination for multiple agency sites in a specific region. 
• Responsible for knowledge of geographic area, payers and services for each site. 
• Responsible to meet agency admission goals along with the agency director and home care liaisons from each agency site.
• Includes random visits to key referral sources.
• Reports to the agency or branch director.

Education and Experience Summary:

• Must possess current RN or LPN license in good standing in state where agency is located
• Minimum 2 years certified home health experience
• Previous experience as an intake coordinator or marketing liaison required
• Must have CPR certification
• Must be able to pass computer proficiency test
