Clinical Coordinator

Description Summary:

• The clinical coordinator is responsible for overall clinical management of Life Care at Home patients and the nurses, therapists and aides who care for our patients.
•  Manages and coordinates patient care utilizing a multidisciplinary team including RN, LPN, PT, PTA, OT, COTA, SLP, MSW and home health aides.
• Supports the company business plan in organizing and implementing the clinical operations of the company.
• The position reports to the agency director.

Education and Experience Summary:

• Must possess current RN license in good standing in the state in which agency is located.
• Minimum 2 years home health experience preferred
• Minimum 2 years recent clinical experience
• Minimum 1 year health care management experience preferred
• Bachelor’s degree in nursing preferred
• Must have CPR certification
• Valid driver’s license and auto liability insurance with reliable transportation to travel to patient homes per patient schedule and agency needs.
