Diabetes presents a serious and growing health concern with one out of every five people age 60 and older having been diagnosed with diabetes. When Life Care at Home joins your health care team, we are all working together toward a common goal: lifetime management of diabetes through education.
The overall goal of the Life Care at Home diabetic management program is to foster self-independence in the management of diabetes, prevent complications of diabetes and decrease emergency care and hospital visits. We bring clinically proven tools to your home and help you put them into practice.
Our program’s centerpiece is the patient education booklet. Written on a fourth grade level, with evidence based practice guidelines and reviewed by a family practice physician, this valuable tool is used to help the diabetic prepare for an emergency, sick day management for when the diabetic has the flu or cold and recommends how to prevent the complications of diabetes.
“Managing your life and your diabetes” is valuable to new diabetic patients and can be tailored for existing diabetic patients who have an exacerbation of the disease related to other illnesses.
